Adult V-VAS™ Knee Orthosis Case Studies: Managing Osteoarthritis of the Knee
Across three featured studies — an ISPO poster, a device design paper, and a UK hospital outcome study, supported by four source documents — the V-VAS™ (Varus Valgum Adjustable Stress) custom knee orthosis was consistently associated with significant pain reduction, improved mobility, and high patient compliance in adults with osteoarthritis of the knee.
Why These Outcomes Matter for Device Selection
Unloader knee orthoses are well established for relieving pain in uni-compartmental knee osteoarthritis, but patient tolerance is often the limiting factor — conventional three-point-pressure designs can concentrate force at the strap or condylar pad, shortening how long a patient will wear the brace. The V-VAS™ design uses a four-point pressure system with a self-aligning polycentric joint instead of a condylar pad, aiming to improve comfort and compliance without sacrificing the offloading effect. The three studies summarized here look at that design from three angles: clinical case outcomes, the underlying biomechanical rationale, and a larger retrospective/prospective outcome review.
About the V-VAS™ Knee Orthosis
The V-VAS™ (Varus Valgum Adjustable Stress) Knee Orthosis is a custom-fabricated, off-loading knee brace manufactured by Anatomical Concepts, Inc. for adults with medial or lateral compartment osteoarthritis of the knee. It is typically indicated for patients who are medically unfit for surgery, too young for joint replacement, or seeking a non-surgical option before or after other interventions.
What the Clinical Evidence Shows
Three studies, spanning a clinical case series, a device design analysis, and a larger hospital outcome review, evaluated the V-VAS™ knee orthosis across pain relief, mobility, and patient satisfaction.
Significant Pain Reduction
In the ISPO poster case series, patients reported a 50–70% reduction in pain on a visual-analogue scale, with 90% of patients across both study groups showing significantly improved pain and mobility. The Inverclyde outcome study found statistically significant pain-score improvement (p<0.01 prospective, p<0.05 retrospective).
Improved Mobility and Walking Distance
The Inverclyde study measured increased walking distance and improved Oxford Knee Scores after bracing (p<0.001 prospective). In the ISPO case series, over 50% of Group 1 patients reported increased mobility in day-to-day tasks, and roughly 10% of Group 2 patients returned to activities such as the gym or hill walking.
High Patient Compliance and Satisfaction
In the ISPO case series, all Group 1 patients declined to return the device when asked at six weeks. The Inverclyde retrospective/prospective study (147 patients) reported an overall satisfaction rate of 78%, with compliance issues more likely in patients with very high BMI or advanced age, and improved by early follow-up and refitting.
Featured Studies
These three featured studies are supported by four source documents. The Inverclyde Hospital outcome study below is documented in two source PDFs — a full paper and a presentation-slide version — covering the same 147-patient study.
Management of Severe OA of the Knee Using a V-VAS™ Customised Off-Loading Knee Orthosis
Munro W.A., Buchan M., Jones D.
An ISPO poster/case study of over 100 V-VAS™ fittings in two patient groups — those with >10° valgus/varus deformity medically unfit for surgery (Group 1), and those too young for joint replacement (Group 2). All Group 1 patients reported 50–70% pain reduction on a visual-analogue scale and refused to return the device at six weeks; over 50% reported increased mobility. Group 2 patients reported similar pain reduction, with about 10% returning to activities like the gym or hill walking. Overall, 90% of patients across both groups showed significantly improved pain and mobility. A sample case (47-year-old male) showed a reduction from 11° varus without the brace to 4° varus while wearing it, on weight-bearing X-ray.
V-VAS™ Orthosis: A New Concept in Unloader Knee Orthosis Design
Whiteside, Joseph W., CO/LO
A design paper explaining how the V-VAS™ differs from conventional single- and double-upright unloader knee orthoses. Rather than a three-point pressure system relying on a strap or condylar pad, the V-VAS™ uses a total-contact thigh and tibial cuff with a self-aligning polycentric joint to create four points of pressure, removing the need for a condylar pad and reducing mechanical joint binding. A featured case (51-year-old male, 220 lb) showed a 5°/5mm improvement in lateral condyle separation on weight-bearing X-ray while wearing the brace, allowing the patient to return to work as a flight attendant.
Clinical Outcome Study of Knee Bracing in the Management of Osteoarthritis of the Knee: A Prospective and Retrospective Study
Buchan M., Walkay S. / Walkay G., Munro W., Buchan M. — Inverclyde Hospital, Scotland UK
A 147-patient outcome study (86 retrospective, 61 prospective) of V-VAS™ knee bracing for osteoarthritis, measuring Oxford Knee Score, visual-analogue pain scores, walking distance, and patient satisfaction. Results showed statistically significant pain-score improvement (p<0.01 prospective, p<0.05 retrospective), improved Oxford Knee Scores (p<0.001 prospective), and increased walking distance, with an overall satisfaction rate of 78%. A small number of patients went on to total knee replacement (6 retrospective, 2 prospective) or high tibial osteotomy (2), and 4 discontinued the brace. The study notes compliance may be lower in patients with very high BMI or advanced age, and improves with patient education and early refitting appointments.

Frequently Asked Questions
What is a V-VAS™ knee orthosis?
The V-VAS™ (Varus Valgum Adjustable Stress) Knee Orthosis is a custom-fabricated, off-loading knee brace used to manage medial or lateral compartment osteoarthritis of the knee. It uses a total-contact thigh and tibial cuff with a self-aligning polycentric joint system to reduce pain and support the affected compartment.
How does the V-VAS™ differ from a traditional unloader knee brace?
Traditional single- or double-upright unloader braces typically use a three-point pressure system involving a strap or condylar pad, which can concentrate pressure and limit how long a patient tolerates the device. The V-VAS™ instead uses four points of pressure through its total-contact cuffs and self-aligning polycentric joint, removing the need for a condylar pad and reducing mechanical joint binding.
How effective is the V-VAS™ for knee osteoarthritis pain relief?
In an ISPO case series of V-VAS™ patients, 90% showed significantly improved pain and mobility. All patients in the deformity-focused group reported 50-70% pain reduction, with similar reductions reported in the second patient group.. A separate 147-patient outcome study found statistically significant pain-score improvement (p<0.01) and improved Oxford Knee Scores (p<0.001) after bracing.
Who is a candidate for the V-VAS™ knee orthosis?
Candidates include patients with more than 10° of valgus or varus knee deformity who are medically unfit for surgery, and younger patients who are not yet candidates for knee replacement. It can be used as a non-surgical option, alongside pain-relieving injection therapy, or as a pre- or post-operative tool.
What do outcome studies show about knee bracing compliance and satisfaction?
A 147-patient retrospective and prospective outcome study of V-VAS™ bracing reported an overall patient satisfaction rate of 78%, along with increased walking distance. Compliance was noted to be more difficult for patients with very high BMI or advanced age, and improved with patient education and early refitting appointments.
